Liability Form

YogaWell Waiver and Release of Liability:

I, the undersigned, acknowledge that I am in good physical health and have no medical condition that would prevent my safe participation in yoga, fitness, or wellness activities offered by YogaWell. I understand that it is my responsibility to consult with a physician before participating in any class, session, or wellness service provided by YogaWell.

I recognize that yoga, fitness, and wellness activities involve inherent risks, including but not limited to personal injury. I voluntarily assume all risks associated with my participation and agree to follow all instructions given by YogaWell instructors, independent contractors, substitute teachers, and guest wellness providers to promote my safety.

 

By signing this waiver, I release and discharge YogaWell, Tracy Schoneman, all YogaWell instructors (including independent contractors, substitute teachers, and guest wellness providers), and any associated parties from any liability, claims, or causes of action arising from my participation in their classes, workshops, or services at any location, whether in-person, virtual, or off-site.

I affirm that I have carefully read and fully understand this waiver and release of liability. By signing below, I voluntarily agree to its terms and acknowledge the importance of practicing mindfully and safely.
